Submission Checklist
- Is your video under two minutes in length?
- Does your video give a specific example (personal story, experience, testimonial, or thoughtful idea) of the benetfits of having access to view your health record and the ability to review what is in your health record?
- Does your video direct viewers to visit www.HealthIT.gov for more information?
- Did you obtain actual signatures for the Video Consent Form from each individual appearing in your video? Note: Only the official form provided by the competition will be accepted.
- Do you have a transcript of all the words that are spoken/sung in your video? If you're using a licensed song in your video, do you own the rights to it?
- Have you reviewed the Judging Criteria, Official Rules, and the Enter a Video page to make sure your submission includes all the required items?
Video Upload Tip
Don't wait until the last minute to upload your video. It could take anywhere from a few minutes to several hours or more, depending on the format of your original video, file size, upload traffic, Internet connection speed, etc. Plan accordingly!
