EHR - A Life Saver (Mary & Jacob)
Mary is 32 years old. Her son, Jacob, is 8 years old. Jacob has asthma and he developed serious complications. He sees multiple specialists for his treatment.
Unfortunately, due to Mary's job, they have to move to a different state. Mary does not want Jacob's treatment to be interrupted. So, before they move, Mary requests Jacob's EHR data and verifies that his latest diagnoses, medications, allergies, test results, etc. are up to date from all of his specialists. When Mary and Jacob moved to the new state, Jacob's new specialists used his EHR data to get his latest medical information and continued his treatment without any interruption.
Mary's use of Jacob's EHR helped prevent a potentially life threatening lapse in Jacob's treatment and successfully treat his serious complications.

The EHR helped my family to:
1. Improve quality of care and patient care experience
2. Increase communication between the doctors, patient and family
3. Coordinate treatment to be interruted
4. Avoid duplication of diagnostic tests
5. Less errors in recording and reporting health history and medications.
6. Provide clear instructions on what is a EHR?.
