What did I find when I reviewed my medical records?

This video is an appeal to improve the accuracy of medical records. When I asked OCR to address the errors in my on-line medical record made by my HMO, they said they need to ensure privacy, but not accuracy. This video reflects the urgency that I feel to address accuracy.
